ELECTRONIC MEDICAL RECORD START-UP GUIDE

The Blackhawk Athletic Trainer’s will begin using a secure computer based record-keeping system (ATS) to record all student-athlete’s medical information. There are a number of forms that will need to be completed online in order to be eligible to participate in athletics at Blackhawk. These documents can only be found within the Athletic Trainer System (ATS). All athletes must login to ATS to enter their demographic information. Once completed, ATS will then give you access to the necessary information to be completed. Returning athletes will be required to login to ATS annually to fill out the necessary forms and update their medical history, emergency contacts, and insurance information. See below for instructions.

Step 1: Go to the website below for the ATS Login Page:

bsd2.atsusers.com

Step 2: All NEW users must enter in information below to begin:

Athlete ID: New

Password: New

Database: atsbsd

Step 3: Fill out all necessary information on demographics page. Create Athlete ID and password. When setting up as a new user, we recommend that you use your last name with your first initial as your Athlete ID and create a new password. If you forget your username or password, contact one of your athletic trainers for assistance. Database is always atsbsd.

Step 4: Once the demographics page has been completed and the system accepts your username and password, the remaining pages may be filled out. Once the forms have been filled out, click on the “check mark” symbol on that page, and save information on demographics page as well. If information needs changed at any time, you may log in to the ATS Athlete Portal at bsd2.atsusers.com and use the Athlete ID and Password that you created.
